ABSTRACT

AIM: To investigate the clinical value of serum vitamin A(Vit A)and basic fibroblast growth factor(bFGF)levels predicting retinopathy of prematurity(ROP).METHODS: Prospective cohort studies. A total of 411 premature or low birth weight infants with gestational age less than 37 wk or birth weight less than 2 500 g who were delivered in Hainan Branch, Shanghai Children's Medical Center Affiliated to Shanghai Jiao Tong University School of Medicine from January 2020 to December 2022 were selected as subjects. The Vit A and bFGF levels in peripheral blood were detected at 7 d and 35 d after birth, respectively.RESULTS: A total of 392 premature infants or low birth weight infants completed clinical study, including 51 cases in stage 1-2 ROP group, 23 cases in stage 3-5 ROP group and 318 cases in the group without ROP. At 7 d postnatal, the serum Vit A(0.44±0.17 μmol/L)and bFGF(0.53±0.16 ng/L)levels in stage 1-2 ROP group were lower than those in the group without ROP(0.50±0.12 μmol/L and 0.63±0.15 ng/L; all P<0.05). The serum Vit A(0.34±0.18 μmol/L)and bFGF(0.44±0.18 ng/L)levels in stage 3-5 ROP group were lower than those in the group without ROP(P<0.05). The serum Vit A and bFGF levels in stage 3-5 ROP group were lower than those in stage 1-2 ROP group(P<0.05). At 35d postnatal, the serum Vit A(0.33±0.19 μmol/L)and bFGF(0.39±0.19 ng/L)levels in stage 3-5 ROP group were lower than those in stage 1-2 ROP group(0.43±0.16 μmol/L and 0.48±0.17 ng/L; all P<0.05). According to the ROC curve drawn by serum Vit A, the AUC value was 0.853, the maximum Youden index was 0.68, the best sensitivity was 73%, and the best specificity was 95%. According to the ROC curve drawn by serum bFGF, the AUC value was 0.828, the maximum Youden index was 0.58, the best sensitivity was 90%, and the best specificity was 68%. According to the ROC curve drawn by serum Vit A combined with bFGF, the AUC value was 0.917, the maximum Youden index was 0.70, the best sensitivity was 70%, and the best specificity was 100%.CONCLUSION: Serum Vit A and bFGF levels are sensitive and effective indicators for predicting ROP. If the serum Vit A or bFGF levels are lower in premature infants or low birth weight infants, it may indicate the higher probability of ROP and its pathological stages. In addition, the clinica value of serum Vit A combined with bFGF in the diagnosis of ROP is higher than that of Vit A or bFGF alone, and the misdiagnosis rate is reduced.

El creciente aumento de daños visuales como, ha llevado a científicos y especialistas a realizar ensayos clínicos con pacientes vulnerables, usando alimentos con nutrientes específicos que aportan al cuidado de las estructuras del globo ocular, pues la nutrición de este órgano se obtiene directamente de la sangre. Objetivo. Analizar los ensayos clínicos con vigencia de 9 años sobre antioxidantes, betacarotenos y otras vitaminas para la prevención de enfermedades oculares. Metodología. Se realizó una revisión bibliográfica bajo con un análisis de 40 artículos, tesis y libros sobres la relación entre la nutrición y el cuidado primario de las estructuras oculares, esto se realizó en tres momentos, búsqueda, clasificación y sistematización de las fuentes. Conclusión. Se confirmó que del 50% de los pacientes que ingirieron placebo no mejoraban su condición anatomo fisiológica en relación aquellos pacientes que sí ingirieron nutrientes y vitaminas específicas, los mismos, mejoraron en un 30% el nivel de agudeza visual. La promoción y prevención como cuidado primario en salud visual es importante y se demuestran los altos porcentajes de buena salud visual en aquellos pacientes que durante su vida tuvieron hábitos alimenticios correctos y nutrición equilibrada, además de ser menormente propensos a adquirir problemas degenerativos visuales, aun teniendo antecedentes hereditarios.


The increasing increase in visual damage has led scientists and specialists to conduct clinical trials with vulnerable patients, using foods with specific nutrients that contribute to the care of the structures of the eyeball, since the nutrition of this organ is obtained directly from the blood. Objective. To analyze 9 years of clinical trials on antioxidants, beta-carotene and other vitamins for the prevention of ocular diseases. Methodology. A bibliographic review was carried out with an analysis of 40 articles, theses and books on the relationship between nutrition and primary care of the ocular structures, this was done in three moments, search, classification and systematization of the sources. Conclusion. It was confirmed that 50% of the patients who ingested placebo did not improve their anatomo-physiological condition in relation to those patients who ingested specific nutrients and vitamins, they improved their visual acuity level by 30%. The promotion and prevention as primary care in visual health is important and the high percentages of good visual health are demonstrated in those patients who during their life had correct eating habits and balanced nutrition, besides being less prone to acquire visual degenerative problems, even if they have hereditary antecedents.


O crescente aumento dos danos visuais tem levado cientistas e especialistas a realizarem ensaios clínicos com pacientes vulneráveis, utilizando alimentos com nutrientes específicos que contribuem para o cuidado das estruturas do globo ocular, uma vez que a nutrição desse órgão é obtida diretamente do sangue. Objetivo. Analisar 9 anos de ensaios clínicos sobre antioxidantes, betacaroteno e outras vitaminas para a prevenção de doenças oculares. Metodologia. Foi realizada uma revisão bibliográfica com análise de 40 artigos, teses e livros sobre a relação entre nutrição e cuidados primários das estruturas oculares, em três etapas: busca, classificação e sistematização das fontes. Conclusões. Confirmou-se que 50% dos pacientes que ingeriram placebo não melhoraram sua condição anátomo-fisiológica em relação aos pacientes que ingeriram nutrientes e vitaminas específicas, os mesmos pacientes melhoraram seu nível de acuidade visual em 30%. A promoção e a prevenção como cuidados primários na saúde visual são importantes e as altas porcentagens de boa saúde visual são demonstradas naqueles pacientes que, durante sua vida, tiveram hábitos alimentares corretos e nutrição balanceada, além de serem menos propensos a adquirir problemas degenerativos visuais, mesmo que tenham antecedentes hereditários.

ABSTRACT Introduction: Renal ischemia (I) could develop due to decreased or ceased blood flow to the kidney in some clinical conditions such as shock, sepsis, and kidney transplantation. The re-supply of blood to the kidney is called reperfusion (R). Ischemia and reperfusion periods can cause severe kidney damage. Objectives: When we examined the I/R molecular progression, antioxidant molecules such as vitamin A seem promising treatment agents. This study aimed to investigate the effects of vitamin A on renal I/R injury. Material and Methods: In the study, 40 Sprague-Dawley male rats were divided into five groups (n=8): the control group, only I/R, I/R+1000, I/R+3000, and I/R+9000 IU/kg of Vitamin A groups. Vitamin A was administrated to each group for seven days via oral gavage. Blood and kidney tissue samples were collected at the end of the experiment. We took blood samples for Superoxide dismutase (SOD), malondialdehyde (MDA), catalase (CAT), blood urea nitrogen (BUN), and creatinine (Cr) levels, and determined their values. The tissue samples were stained with hematoxylin/eosin to examine the renal changes histopathologically and stereologically under a light microscope. Results: Histopathological changes caused by I/R were decreased with vitamin A administration in a dose-dependent manner (p<0.05). Vitamin A administration decreased MDA levels and increased SOD and CAT activities (p<0.05). The most effective dose among treatment groups was 9000 IU/kg. There was no significant difference between the controls and all other groups regarding BUN and Cr concentrations. Conclusions: Consequently, administration of vitamin A after renal I/R reduced the histological damage and ameliorated the antioxidant state. These results showed that vitamin A could be a promising agent in treating I/R-induced acute kidney injury.


RESUMEN Introducción: La isquemia renal (I) puede desarrollarse debido a la disminución o interrupción del flujo sanguíneo al riñón en algunas condiciones clínicas como shock, sepsis y trasplante renal. El reabastecimiento de sangre al riñón se denomina reperfusión (R). Tanto la isquemia como los períodos de reperfusión pueden causar graves daños renales. Objetivos: Cuando examinamos la progresión molecular I/R, las moléculas antioxidantes como la vitamina A parecen agentes de tratamiento prometedores. El objetivo de este estudio fue investigar los efectos de la vitamina A sobre la lesión renal I/R. Material y Métodos: En el estudio, 40 ratas macho Sprague-Dawley se dividieron en 5 grupos (n=8) como: control, solo I/R, I/R+1000, I/R+3000 e I/R+9000 UI/kg de la Vitamina A. La vitamina A se administró a cada grupo durante 7 días por vía oral forzada. Al final del experimento se recolectaron muestras de sangre y tejido del riñón. A partir de muestras de sangre se determinaron los niveles de superóxido dismutasa (SOD), malondialdehído (MDA), catalasa (CAT), nitrógeno ureico en sangre (BUN) y creatinina (Cr). Las muestras de tejido se tiñeron con hematoxilina/eosina y los cambios en la histología renal se examinaron histopatológicamente y estereológicamente al microscopio de luz. Resultados: Los cambios histopatológicos causados por I/R disminuyeron con la administración de la vitamina A de manera dependiente de la dosis (p<0,05). La administración de la vitamina A disminuyó los niveles de MDA, aumentó las actividades de SOD y CAT (p<0,05). La dosis más eficaz entre los grupos del tratamiento fue de 9000 UI/kg. No hubo una diferencia significativa entre el grupo control y todos los demás grupos con respecto a las concentraciones de BUN y Cr. Conclusiones: Consiguientemente, la administración de la vitamina A, después de I/R renal, redujo el daño histológico y mejoró el estado antioxidante. Estos resultados mostraron que la vitamina A puede ser un agente promisorio en el tratamiento de la lesión renal aguda (LRA) inducida por I/R.

Abstract Introduction Laryngeal leukoplakia corresponds to a white lesion in the mucosa developed by the deposit of keratin in the epithelium, potentiated by chronic smoking. It is considered a preneoplastic lesion. Surgery is the most adopted treatment; however, non-surgical treatment is advocated by some authors. Objective To evaluate the effectiveness of vitamin A in the management of vocal fold leukoplakia. Methods Patients with videolaryngoscopy diagnosis of vocal fold leukoplakia were selected. The endoscopic images were photographed and with the aid of the ImageJ software (National Institutes of Health, Bethesda, MD, USA), the proportion of the size of the leukoplakia was calculated. Eligible patients were prescribed 50,000U of vitamin A, twice daily for 2 months, at which point videolaryngostroboscopy was repeated for comparative analysis between pre and posttreatment. The efficacy of the treatment was classified as: outcome I - complete improvement of the lesion, outcome II - partial improvement, and outcome III - no difference or increased lesion size. Results Fifteen patients (eight women, seven men) were included, six of whom had bilateral lesions. Smoking was reported in 86.8% of patients. Complete improvement of the lesion was found in 7 cases (33.4%, outcome I), partial improvement in 6 (28.6%, outcome II), and worsening of the injury in 8 (38.1%, outcome III). Of the latter, 6 underwent microsurgery; histopathology indicated absence of dysplasia in 3, and mild dysplasia in 3. Conclusions In this study, the treatment with vitamin A at a dose of 100,000 IU daily for 2 months was effective in reducing the laryngeal leukoplakia size in 62% of cases.

ABSTRACT

Vitamin A is essential for the health of the mother as well as for the health and development of the fetus. Vitamin A deficiency has affected 19 million pregnant women with the highest burden found in the WHO regions of Africa and South-East Asia. Vitamin A is available in multiple vitamin formulations for prenatal care in some countries. When provided alone, the compounds most commonly used are retinyl palmitate and retinyl acetate in tablet form or oil-based solutions. Hypertension is common in pregnancy and causes high maternal mortality. This includes gestational hypertension, preeclampsia, severe preeclampsia, and pregnancy with chronic hypertension. Preeclampsia is a high-mortality disease among the common complication of hypertensive disorder of pregnancy. in particular, severe preeclampsia possess a serious threat to the safety of mothers and children, and there are great difficulties in the treatment of hypertensive disorders of pregnancy during clinical work. therefore, we are adequate in dealing with hypertensive disorders of pregnancy. Any adverse pregnancy outcomes associated with hypovitaminosis D should be accessed through the perspective of immune dysregulation both at the systemic and placental levels. It signifies the supplementation of vitamin D in pregnancy have a role in the improvement of maternal hypertensive complication and improve the fetal outcome.

Background: Ocular involvement is relatively common in malnourished children, and the consequences of malnutrition on the eye are frequently more severe during the formative years of life. This is a significant cause of avoidable blindness that has to be addressed with affordable methods. This study aims to determine the nature, extent, and seriousness of ocular manifestations in malnourished children and to evaluate the current prevalence given improving nutritional parameters through increased public awareness, education, and knowledge of vaccination and nutrition. Materials and Methods: This study was carried out in the department of ophthalmology of a tertiary eye care center for 1 year. Eye camps were held close by in urban shantytowns. We considered 518 kids between the ages of 6 months and 5 years who lived in those slums. The patient was subjected to a general, systemic, and ocular examination, including a vision and fundus examination. Results: A total of 100 children aged 6 months and 5 years (60 months) who were severely critically malnourished were enrolled and given an eye examination. In this study, 72% of very acutely malnourished children had ocular involvement. Conjunctival pallor was the most general observation in the anterior segment in 179 of the eyes (71.5% of the eyes). Retinal hemorrhage was the most pervasive posterior segment findings in 19 eyes (36.8%). Conclusion: This study is aimed to determine how frequently ocular symptoms occurred in malnourished children who visited tertiary eye care facilities. A vital target population that requires comprehensive screening to avoid vitamin A deficiency (VAD) and detect eye problems early is preschoolers. Every child should also receive an immunization and vitamin A prophylaxis. Low socioeconomic position, illiterate parents, and inadequate sanitation were determined to be the key risk factors for VAD.

ABSTRACT

Resumo Objetivou-se avaliar o Programa Nacional de Suplementação de Vitamina A em Minas Gerais. Utilizou-se método misto sequencial explanatório. Na abordagem quantitativa, utilizou-se instrumento multidimensional segundo componentes de suplementação da vitamina A e educação alimentar e nutricional. Usou-se uma matriz de análise com parâmetros para definir a implantação como adequada, parcialmente adequada, não adequada e crítica. Na abordagem qualitativa, foram utilizadas entrevistas semiestruturadas. A dimensão processo foi melhor avaliada que a estrutura, apresentando grau de implantação de 84,6% e 78,5%, respectivamente. As fortalezas do programa incluem: planejamento da suplementação, alcance de metas, registro de informações, apoio ao aleitamento materno e atuação dos agentes comunitários de saúde. Entre as fragilidades estão: fragmentação do trabalho, limites na análise das informações, acesso às ações do programa, implantação de ações educativas, ausência ou insuficiência de nutricionistas e capacitações. A realidade do programa não contemplou, em sua totalidade, ações de promoção da saúde, sendo o foco a suplementação da vitamina A. É imperativo implementar ações de educação alimentar e nutricional para o enfrentamento da deficiência de vitamina A.


Abstract The present study aimed to evaluate the National Vitamin A Supplementation Program in Minas Gerais, adopting the mixed sequential explanatory method. The quantitative approach adopted the multidimensional instrument per the components of vitamin A supplementation and food and nutrition education. We employed an analysis matrix with parameters to define the implementation as adequate, partially adequate, inadequate, and critical. We used semi-structured interviews in the qualitative approach. The "process" dimension was better evaluated than the "structure", with an implementation level (IL) of 84.6% and 78.5%, respectively. The Program's strengths include supplementation planning, achieving goals, recording information, supporting breastfeeding, and the performance of Community Health Workers. Weaknesses are fragmented work, analysis of information limits, access to the Program's actions, implementation of educational actions, and lack of or insufficient availability of nutritionists and training. The Program's reality only partially considered health promotion actions and focused on vitamin A supplementation. Implementing food and nutritional education actions is imperative to face vitamin A deficiency.

ABSTRACT

Abstract: This manuscript aims to report the nutrition transition in Brazilian children under 5 years old from 2006 to 2019. Microdata from the Brazilian National Survey on Demography and Health of Women and Children (PNDS 2006) and the Brazilian National Survey on Child Nutrition (ENANI-2019) were analyzed. The indicators considered were: micronutrient status (anemia and vitamin A deficiency), anthropometric status (stunting and excessive weight), and breastfeeding practice (exclusive breastfeeding among children < 6 months and continued breastfeeding among children 12-23 months). We also analyzed minimum dietary diversity (MDD), consumption of ultra-processed foods, consumption of meat or eggs, and not consuming fruits or vegetables in children 6-59 months of age only for ENANI-2019. Equiplot charts were generated according to geographic region, maternal schooling level, and maternal race/skin color. From 2006 to 2019, the prevalence rates of anemia and vitamin A deficiency decreased from 20.5% to 10.1% and 17.2% to 6%, respectively. The prevalence of stunting remained at 7%, and excessive weight rates increased from 6% to 10.1%. The prevalence of exclusive breastfeeding among children < 6 months increased from 38.6% to 45.8%, and of continued breastfeeding among children 12-23 months from 34.6% to 43.6%. In 2019, 61.5% of children achieved the MDD, 88.8% consumed ultra-processed foods, 83.1% consumed meat or egg, and 25.7% did not consume fruits or vegetables the day before the survey. Trends of decreased micronutrient deficiencies, increased breastfeeding, and excessive weight rates, as well as reductions in disparities related to geographic region, maternal schooling level, and maternal race/skin color, were observed for most of the indicators.


Resumo: Buscamos reportar a transição nutricional em crianças brasileiras menores de 5 anos de idade entre 2006 e 2019. Foram analisados microdados da Pesquisa Nacional de Demografia e Saúde da Criança e da Mulher (PNDS 2006) e do Estudo Nacional de Alimentação e Nutrição Infantil (ENANI-2019). Foram considerados os seguintes indicadores: status de micronutrientes (anemia e deficiência de vitamina A), estado nutricional antropométrico (excesso de peso e baixa estatura) e a prática de aleitamento materno (aleitamento materno exclusivo entre crianças < 6 meses e aleitamento materno continuado entre crianças de 12-23 meses). Analisamos a diversidade alimentar mínima (DAM), o consumo de alimentos ultraprocessados, de carne ou ovos e o não consumo de frutas ou hortaliças apenas para o ENANI-2019 em crianças de 6-59 meses de idade. Equiplots foram gerados de acordo com a região geográfica, escolaridade e raça/cor da pele maternas. Entre 2006 e 2019, as prevalências de anemia e deficiência de vitamina A diminuíram de 20,5% para 10,1% e de 17,2% para 6%, respectivamente. A prevalência de déficit de estatura manteve-se em 7% e a de excesso de peso aumentou de 6% para 10,1%. A prevalência de aleitamento materno exclusivo entre crianças < 6 meses aumentou de 38,6% para 45,8% e a de aleitamento materno continuado entre crianças de 12-23 meses aumentou de 34,6% para 43,6%. Em 2019, 61,5% das crianças atingiram a DAM, 88,8% consumiram alimentos ultraprocessados, 83,1% consumiram carne ou ovos e 25,7% não consumiram frutas ou hortaliças no dia anterior à pesquisa. En 2019, el 61,5% de los niños alcanzaron DDM, el 88,8% consumieron alimentos utraprocesados, el 83,1% consumieron carne o huevos y el 25,7% no consumieron frutas o verduras el día anterior a la encuesta. Observamos tendencias de disminución de las deficiencias de micronutrientes, un aumento de la lactancia materna y sobrepeso y reducciones en las disparidades regionales, de escolaridad y de raza/color de la piel de la madre para la mayoría de los indicadores.

Objective@#To investigate the status of physical development, vitamin A and vitamin D among primary and middle school students in poor rural areas in Hubei, to evaluate the implementation effectiveness of the Nutrition Improvement Program for Rural Compulsory Education Students (abbreviated to the program) from 2012 to 2021.@*Methods@#In Enshi City, a pilot monitoring county of the program in Hubei, and a total of 2 554 students from one primary school and one junior middle school were randomly selected. During September to October in 2012-2021, height was measured, and fasting venous blood was collected from students, and serum vitamin A and vitamin D were detected.@*Results@#From 2012 to 2021, height, serum vitamin A and serum vitamin D concentration increased by 2.7 cm, 96.7 μg/L and 8.6 ng/mL. Growth retardation rate decreased by 1.0%. The subclinical and clinical deficiency of serum vitamin A decreased by 37.3% and 6.2%. The subclinical and clinical deficiency of serum vitamin D decreased by 45.7% and 8.5%. Height was positively correlated with serum vitamin A status and negatively correlated with serum vitamin D status ( r =0.08,-0.08, P <0.01).@*Conclusion@#The pilot counties of the program in Enshi, has witnessed improvement in height, serum vitamin A and vitamin D status among primary and secondary students. However, future efforts remains essential as subclinical deficiency of vitamin A and vitamin D are noted in some students. Serum vitamin A is positively associated with height, and vitamin D is negatively associated with height.Considering the correlation between serum vitamin A and vitamin D status with height, continued efforts should be implemented in poor rural areasto strengthen targeted nutrition intervention for students in Hubei.

Objective:High performance liquid chromatography-tandem mass spectrometry (HPLC-MS/MS) was used to quantify the levels of vitamins A, D and E in pregnant women during the second trimester, and to investigate the change trends of serum vitamins A, D and E levels during pregnancy.Method:A total of 720 pregnant women with an average age of (29.7±4.4) years and 12-22 weeks of gestation were included from October 1, 2021 to October 30, 2022 in the obstetrics department of the People′s Hospital of Wuhan University. The concentrations of vitamins A, D and E were determined by HPLC-MS/MS. The concentration levels of each group were statistically analyzed and the deficiency rate were calculated.Results:The distribution range of vitamin A, D and E (95% CI) was 0.74-2.74 μmol/L, 2.88-25.37 ng/ml and 6.18-35.08 μmol/L, with the deficiency rates were 9.30%, 93.76% and 35.83%, respectively. Vitamin A, D and E levels in the twin group were (1.67±0.51) μmol/L, (13.18±7.44) ng/ml and 11.97 (8.85, 14.60) μmol/L, respectively. They were significantly higher than those in the singlet group (1.45±0.36) μmol/L, (10.87±5.26) ng/ml and 10.46 (6.99, 14.11) μmol/L, with statistical significance by independent sample t-test ( P<0.001). The concentration of vitamin D in the lower BMI group (<22 kg/m 2) was (12.54±5.74) ng/ml, significantly higher than that in the fat group (≥22 kg/m 2) (10.46±4.90) ng/ml, and the rank-sum test was statistically significant ( P<0.001). Conclusion:In this study, the levels of three vitamins were monitored in mid-pregnancy using HPLC-MS/MS, and the changes of serum vitamin A, D, and E levels during pregnancy were analyzed.

ABSTRACT

The silent epidemic of micronutrient deficiencies (MNDs) continues to be a major public health challenge in the developing world, including India. The prevalence of iron, iodine, zinc, vitamin A and folate deficiencies is alarmingly high worldwide. India is additionally facing a high prevalence of vitamin D and B12 deficiencies. To combat the hidden epidemic of MNDs, various governments around the world have mostly relied on supplementation or fortification-based interventions. India launched salt iodization programme in 1962 and vitamin A and iron-folate supplementation programmes in 1970. Yet, even after decades of these programmes, MNDs are still widespread in the country. Due to slow progress in alleviating the burden of most MNDs, the Government of India aims to scale up fortification-based intervention programmes. However, there are safety and effectiveness concerns with such approaches. Hence, overdependence on supplementation and fortification alone may be counterproductive. Instead, food based dietary diversification approach can be the way forward. In this article, we list the common MNDs in India, evaluate major policy interventions, discuss concerns pertaining to fortification and suggest the need for a concurrent food-based approach, in particular dietary diversification, as a long-term and sustainable strategy to address population-based MNDs.

Abstract Objective: To determine the prevalence of vitamin A deficiency (VAD) and serum concentrations of retinol, correlating them with IGF-1 concentrations in preschoolers with DS. Methods: Cross-sectional study was conducted on 47 children with DS aged 24 to 72 months, in Ribeirão Preto, Brazil. VAD was determined by the relative dose-response (RDR) test. Retinol serum concentration ≤ 0.70 μmol/L and IGF-1 serum concentration below the 3rd percentile for sex and age were considered to represent deficiency. C-reactive protein (CRP) was determined at the beginning of the study. Weight, height, and information about fever and/or diarrhea were obtained at the beginning of the study. Results: VAD prevalence was 25.5% (12/47), and 74.5% (35/47) of the children had deficient retinol before the intervention. CRP was not associated with VAD. Mean IGF-1 were 103.5 ng/mL (SD = 913) for the group with VAD and 116.3 ng/mL (SD = 54.9) for the group with no VAD (p-value = 0.85); 8.5% (4/47) of the children showed deficient IGF-1, but without VAD. No association was observed between VAD and IGF-1 deficiency. A moderate positive correlation was observed between pre-intervention retinol and IGF-1 (ρ = 0.37; p-value = 0.01). Conclusion: a high prevalence of VAD and deficient retinol was observed and there was a positive correlation between serum retinol and IGF-1.

OBJECTIVES@#To investigate the levels of fat-soluble vitamins A, D, and E in children with obesity and their influencing factors.@*METHODS@#A total of 273 children with obesity who attended the Department of Clinical Nutrition, Xi'an Children's Hospital, from January 2019 to April 2021 were enrolled as the obesity group. A total of 226 children with normal body weight who underwent physical examination during the same period were enrolled as the control group. Anthropometric parameters and body composition were measured for both groups, and the serum concentrations of vitamins A, D, and E were also measured.@*RESULTS@#Compared with the control group, the obesity group had significantly higher serum levels of vitamin A [(1.32±0.21) μmol/L vs (1.16±0.21) μmol/L, P<0.001] and vitamin E [(9.3±1.4) mg/L vs (8.3±1.2) mg/L, P<0.001] and a significant reduction in the level of 25-hydroxyvitamin D [(49±22) nmol/L vs (62±24) nmol/L, P<0.001]. In the obesity group, the prevalence rates of marginal vitamin A deficiency, vitamin D deficiency/insufficiency, and vitamin E insufficiency were 5.5% (15/273), 56.8% (155/273), and 4.0% (11/273), respectively. After adjustment for body mass index Z-score and waist-to-height ratio, serum vitamin A level was positively correlated with age (P<0.001), while vitamins E and 25-hydroxyvitamin D levels were negatively correlated with age in children with obesity (P<0.001). After adjustment for age, the serum levels of vitamin A, vitamin E and 25-hydroxyvitamin D were not correlated with degree of obesity, percentage of body fat, and duration of obesity in children with obesity, while the serum levels of vitamins A and E were positively correlated with waist-to-height ratio (P<0.001).@*CONCLUSIONS@#There are higher serum levels of vitamins A and E in children with obesity, especially in those with abdominal obesity, while serum vitamin D nutritional status is poor and worsens with age. Therefore, vitamin D nutritional status should be taken seriously for children with obesity, and vitamin D supplementation should be performed when necessary.

ABSTRACT Primary biliary cirrhosis is a rare progressive autoimmune liver disease that causes chronic cholestasis. Of patients with primary biliary cirrhosis, 75% develop secondary Sjogren syndrome and could develop vitamin A deficiency. Here, we report the case of a patient with primary biliary cirrhosis who developed a secondary Sjogren syndrome and vitamin A deficiency, which led to severe and unusual eye involvement with multiple and recurrent spontaneous corneal perforations. Corneal perforations in patients with primary biliary cirrhosis and secondary Sjogren syndrome are rare but devastating complications, in contrast to other eye clinical manifestations of the disease.


RESUMO A cirrose biliar primária é uma doença hepática autoimune progressiva rara que causa colestase crônica. 75% dos pacientes com Cirrose Biliar Primária desenvolvem Síndrome de Sjögren Secundária, e podem também desenvolver deficiência de vitamina A. Aqui, relatamos um paciente com Cirrose Biliar Primária que desenvolveu Síndrome de Sjögren Secundária e deficiência de vitamina A, levando a envolvimento ocular grave e incomum com perfurações espontâneas múltiplas e recorrentes da córnea. Perfurações da córnea em pacientes com Cirrose Biliar Primária e Síndrome de Sjögren Secundária são complicações raras, mas devastadoras, em contraste com outras manifestações clínicas oculares da doença.

Resumo Introdução A educação alimentar e nutricional é um campo de conhecimento e de prática contínua e permanente, que objetiva promover hábitos alimentares saudáveis. Objetivo Apresentar desafios e fortalezas das ações de educação alimentar e nutricional realizadas no Programa Nacional de Suplementação de Vitamina A em municípios de Minas Gerais, no contexto da Atenção Primária à Saúde. Método Estudo transversal realizado em 105 municípios. Participaram 202 profissionais, enfermeiros e nutricionistas, que responderam a um questionário on-line. Resultados Falta de adesão, dificuldade de acesso à população, ausência de referência técnica e do profissional nutricionista, falhas nas atividades em grupos, apoio insuficiente da gestão municipal, da comunidade e da liderança da referência do Programa foram algumas das dificuldades relatadas quanto à realização das ações da educação alimentar e nutricional. Como ponto positivo, foi identificada a realização de ações de apoio à amamentação exclusiva. Conclusão Os resultados apontam para importantes elementos para subsidiar a tomada de decisão na Atenção Primária à Saúde em relação às ações educativas no campo da alimentação e nutrição.


Abstract Background Food and nutrition education is a field of knowledge and continuous and permanent practice, which aims to promote healthy eating habits. Objective to present challenges and strengths of food and nutrition education actions carried out in the National Vitamin A Supplementation Program in municipalities of Minas Gerais, in the context of Primary Health Care. Method This is a cross-sectional study carried out in 105 municipalities. A total of 202 professionals, nurses, and nutritionists participated, who answered an online questionnaire. Results Lack of adherence and difficulty in accessing the population, lack of technical and professional nutritionist reference, failures in group activities, insufficient support from the municipal management, the community, and the leadership of the Program's reference were some of the difficulties reported in terms of achievement actions of food and nutrition education. As a positive point, actions to support exclusive breastfeeding were identified. Conclusion The results point to important elements to support decision-making in primary health care about educational actions in the field of food and nutrition.

ABSTRACT Beauregard sweet potato is an orange color crop rich in β-carotene, a precursor of vitamin A. β-carotene improve immunity and decrease of incidence of degenerative diseases. The objective of this work was to introduce sweet potato in the diet through of some food products, such as bread, cake and sweets. The effect of processing on the chemical composition, as well as the quantification of phenols and total carotenoids, antioxidant activity, and sensory acceptability were evaluated. Cakes and coconut sweets presented higher levels of carotenoids. Higher phenolic and antioxidant activity were verified in cocoa sweets. All foods had good acceptability: 86% for bread, 83.3% for cake and 84,4%, and 86% for coconut and cocoa sweets, respectively. Servings of 95g of roasted sweet potato, 330g of bread, 182g of cake, 187g of coconut sweet and 391g end of cocoa sweet, would provide the recommended daily intake of vitamin A for children 10 years and older. It was concluded that the elaboration of foods containing sweet potato is a viable alternative to inserting biofortified foods into the human diet.


RESUMEN Los camote Beauregard son genotipos de color naranja, ricos en β-caroteno, precursores de la vitamina A. El β-caroteno ayuda a mejorar la inmunidad y reducir el riesgo de enfermedades degenerativas. El objetivo del trabajo fue introducir el camote en la dieta a través del pan, pasteles y dulces tipo postre. Se evaluó el efecto del procesamiento sobre la composición química, así como la cuantificación de fenoles y carotenoides totales, actividad antioxidante y aceptabilidad sensorial. Las tortas y caramelos de coco mostraron mayores niveles de carotenoides y cacao, mayor actividad fenólica y antioxidante. Se confirmó una buena aceptabilidad, en la cual el pan presentó 86%, el torta 83,3%, y los dulces tipo postre de coco y de cacao 84,4% y 86% respectivamente. Las porciones de 95 g de batatas al horno, 330 g de pan, 182 g de torta, 187 g y 391 g de dulce de coco y dulce de cacao respectivamente, proporcionarían la ingesta diaria recomendada de vitamina A para niños de 10 años en adelante. Se concluyó que la preparación de alimentos que contienen batatas es una alternativa viable a la inclusión de alimentos biofortificados en la dieta humana.

ABSTRACT

Objective:To detect serum levels of vitamin A (Vit A), vitamin D(Vit D)25-hydroxy vitamin D[25-(OH)D] and vitamin E(Vit E) in children aged 0-6 years in Tibetan Plateau of Garzi Prefecture, thus providing references for physical examinations and prevention of 4 key diseases (rickets, malnutrition anemia, pneumonia and diarrhea) in children in plateau areas by relevant government departments.Methods:A total of 2 122 children who participated in physical examination in 12 townships of Xiangcheng County and 14 townships of Daocheng County, Garzi Tibetan Autonomous Prefecture, Sichuan Province from April 2017 to April 2019 with 0-6 years old were recruited for surveying physical measurements and collection of venous blood.Serum Vit A and Vit E levels were detected by high performance liquid chromatography.Serum levels of 25-(OH)D were detected by high performance liquid chromatography tandem mass spectrometry.The relationship between Vit A, Vit E and 25-(OH)D levels with the gender, age, seasonal change and altitude was analyzed.Results:The serum Vit A level, subclinical Vit A deficiency rate and marginal vitamin A deficiency rate were(1.05±0.27) μmol/L, 8.15%(173/2 122 cases) and 45.99%(976/2 122 cases), respectively in 2 122 children with 0-6 years old.There were significant differences in the serum Vit A level, the subclinical Vit A deficiency rate and the marginal vitamin A deficiency rate in children with different ages, seasons and altitudes (all P<0.05). The serum level of 25-(OH)D and 25-(OH)D deficiency rate insufficient rate were (24.65±6.45) ng/L, 6.03%(128/2 122 cases) and 16.59%(352/2 122 cases), respectively.There were significant differences in the serum level of 25-(OH)D, 25-(OH)D deficiency rate and 25-(OH)D insufficient rate in children with different ages and seasons (all P<0.05). The mean serum Vit E level, Vit E deficiency rate and Vit E insufficient rate were (7.81±1.74) mg/L, 2.78%(59/2 122 cases) and 29.59%(628/2 122 cases), respectively.There were significant differences in serum Vit E level, Vit E deficiency rate and Vit E insufficient rate in children with different ages and seasons (all P<0.05). The mean serum levels of Vit A and Vit D remained the lowest before the age of 1 year, and their deficiencies at this age were the most significant.The mean serum level of Vit E remained the lowest in >1-2 years old, and its deficiency and insufficient at this age were the most significant.Vit A, D and E levels were significantly affected by seasonal changes, which were significantly higher in the summer than in the spring, autumn and winter.In addition, Vit A and 25-(OH)D were significantly affected by the altitude, which were the lowest above 4 km altitude. Conclusions:The overall serum levels of Vit A, 25-(OH) D and E in children with 0-6 years old in Tibetan Plateau areas of Ganzi Prefecture are lower than those in plain areas.Vit A, 25-(OH) D and Vit E levels significantly differed in the age, season and altitude, which are related to the lack of local resources, insufficient maternal nutrition during pregnancy and insufficient intake after birth, as well as temperature and light caused by changes in local seasons and altitude.Therefore, it is necessary to make reasonable supplements during pregnancy to prevent vitamin deficiency.

ABSTRACT

Objective To analyze the nutritional status and growth and development of children aged 1 to 12 years old, and to provide guidance for subsequent child health care work. Methods A total of 514 children aged 1 to 12 years old underwent physical examination in our hospital were enrolled, data including weight, height, bone mineral density, serum vitamin A, vitamin D and blood trace elements were detected, thereafter, Pearson correlation analysis was used to discuss the correlation between children's growth and development and serum trace elements. Results Among the 514 children in this study, there were 224 boys and 290 girls, including 86 children in early childhood group, 165 children in preschool age group and 263 children in school age group. The prevalence of vitamin A deficiency was 7.98% (41/514), with a significant difference among the different age groups, including 22.09% (19/86) in early childhood group, 7.88% (13/165) in preschool age group, and 4.56% (12/263) in school age group. The height and weight of children with vitamin A deficiency were lower than those of children in the same age group, and there was a positive correlation between serum vitamin A concentration and height and weight of children (P<0.05). The detection rate of vitamin D deficiency was 12.06% (62/514), with 16.07% (36/224) in girls, higher than 8.97% (26/290) in boys. The height of children with vitamin D deficiency was significantly lower than that of children with normal vitamin D levels (P<0.05), and the incidence of bone mineral deficiency was significantly higher than that of that with normal vitamin D levels. Serum vitamin D concentration was positively correlated with height in children (P<0.05) and negatively correlated with bone mineral deficiency (P<0.05). In addition, there were 51 children (9.92%) with hypocalcemia. Serum calcium levels were significantly lower in boys than in girls (P<0.05), and the incidence of hypocalcemia was significantly higher in boys than in girls (P<0.05).Serum calcium level was positively correlated with height (P<0.05), and negatively correlated with bone mineral deficiency (P<0.05). There were 35 cases (6.81%) of children with hypoglycemic zinc and 32 cases (6.23%) of children with hypoglycemic iron, and there was a positive correlation between serum zinc and iron levels and children's height and weight (P<0.05). Conclusions Vitamin A deficiency is more serious in younger children than in older children, and vitamin A deficiency affects children's height and weight; vitamin D deficiency is more serious in girls than in boys, and vitamin D deficiency affects children's bone density. Vitamin D supplementation and outdoor activities for girls are recommended. In addition, deficiencies in trace vitamins, including blood calcium, zinc and iron, will also affect children's height and weight, and a proper diet is recommended to supplement the body with essential elements.

Background@#As the SARS-CoV-2 virus (COVID-19) continues to affect the capacity of the health system to deliver essential nutrition and health services, many countries, especially the low- and middle-income countries, including the Philippines, are beset with addressing undernutrition and micronutrient deficiencies among children. @*Objective@#The rapid nutrition assessment survey (RNAS) was conducted in selected areas in the Philippines to provide evidence of the effect of the COVID-19 pandemic on children's participation in the nutrition services and their mothers' knowledge level on nutrition. @*Methodology@#The RNAS was a cross-sectional survey conducted through phone interviews in nine (9) selected areas in the Philippines. A total of 7,092 children aged 0 to 12 years were included in the survey, of which 5,239 mothers and caregivers were interviewed regarding nutrition program participation of their children, and their knowledge on nutrition. @*Results@#A significant decline in children's participation in the Operation Timbang (OPT) Plus (83.0% vs. 51.1%), Vitamin A supplementation (65.4% vs. 54.9%), supplementary feeding (21.6% vs. 11.9%), and deworming (60.4% vs. 11.9%) was recorded during the COVID-19 pandemic based on RNAS as compared to the pre-pandemic period using the 2019 Expanded National Nutrition Survey (ENNS). The most commonly cited reasons for health service disruptions include: child did not visit health centers due to fear of infection, community health workers did not visit the child at home, and no available health workers in the facility. Stunting among children was more likely to be perceived by food insecure (19.7% vs 14.4%; p <0.001) and poor (22.1% vs 16.6%; p<0.000) mothers or caregivers than their counterpart mothers from food secure and rich households. @*Conclusion@#With prolonged limited access to health and nutrition services, children, particularly those from poor households, are at risk of hunger, malnutrition, and consequently, of not developing their full potential. Findings can guide efforts to ensure the continuity of essential health and nutrition services towards improving maternal knowledge and promotion of child health and nutrition during the COVID-19 outbreak and crisis recovery period.
